Customer Service Surveys

As part of any Comprehensive Quality program customer feedback is necessary. We all receive feedback from our Emergency Service agencies and Public Users particularly if the have had an undesirable experience. I do know that some agency mail survey's to the public, I was wondering if anyone uses an online survey like "SurveyMonkey" to gather information from the public and has it been successful.
