Trainee's Progress Chart/ Tracking Sheet

I am a trainer for Fire/Rescue and need a chart to show trainee's progress.  I was attempting to design on eon Excel using our DOR criteria as a method of graphing progress in specific areas.  If anyone would be willing to assist or post one so I could review and get ideas about format and such I would greatly appreciate it.

Our typical training goes through 3 Phases and last anywhere up to 1 year.

My goal is to create a sheet that will provide a quick glance of trainee's progress and make it easy to identify if certain areas are in need of emphasis.

Was anyone able to get the Excel tracking sheet. I have the DOR but I have seen the tracking sheet that goes along with it.

There is an Electronic Trainee Tracking Sheet out there. I have not been able to get a copy of it yet, I have been searching. Robbie McCormick with REDA in Artesia, NM has one. She was my instructor for a CTO class at the beginning of the month and she went over it with us. It is really awesome. I can not wait to start using it. It is an Excel document.
